[R] design/HMISC packages

The Windows packages of Hmisc and Design are being fixed and I will announce the availability of a new version soon.  Xiao Gang Fan has found a problem with the .zip files that he thinks resulted from having R open in another session at the time he was building the packages.  

The Linux/Unix/MacOSX versions have been updated and now most of the Cox model functions are working in Design.
